The original work was first published in 1985 by Irvington Publishers, Inc. in a single volume, and then reissued in a two-volume paperback set in 1989 and 1992. Havens recently discovered that the order of the presentation of the material was reversed in the two-volume set. Fortunately, Crown House Publishing Ltd. has now republished the text in its original order, again in a single- volume format, which also includes the addition of several references inadvertently omitted from earlier editions.
